The evaluation of treatment data and the analysis of the data obtained or to be obtained from it have become an important tool of medical research and will become even more important in the future for the benefit of the patient. In order to understand diseases, their causes and courses, it is important to learn more about the underlying biological processes. For this reason, we would like to use your pseudonymized disease data (pseudonymized means that the name is replaced by a letter or number code) for research purposes in cooperation with universities and research institutes, in order to develop or improve prevention programs and means for effective disease treatment within the framework of scientific studies. For this reason, we ask our patients, and therefore you as well, whether you are willing to provide us with data and, if blood is drawn from you as part of your treatment, to make it available for research. Your consent to the use of the biomaterial and your health data is voluntary. If blood is drawn from you and you consent to the provision of this biomaterial, approximately 3-4 ml of additional blood will be drawn in a separate tube. To the extent that you do not wish to participate or wish to withdraw your consent at a later date, you will not be disadvantaged.
I agree that Avi Medical GmbH may collect
- collect and store personal data from me, in particular information about my health,
- to extract further personal data, in particular information about my health, from my medical records,
- collects and stores further personal data, in particular information about my health, which I have submitted via the Avi Medical App or which is collected via other apps and shared by means of the Avi Medical App
- and analyzes the data together with my biomaterials (e.g., blood drawn) in a pseudonymized (i.e., coded) manner and publishes the results anonymously in a scientific study.
My biomaterials and data may be kept for up to 10 years. After that, my biomaterials will be destroyed and personal data will be deleted.
They may be passed on pseudonymized to universities, research institutes and researching companies for medical research purposes.
I am aware that the processing of health data, to which I hereby consent, serves scientific research in the form of, among other things, determining the influence of risk factors and therapies on the occurrence of diseases and their course. I am also aware that in particular the blood values collected will be analyzed to determine whether they can be correlated with a disease, its severity and the prognosis of its course. I know that my participation in the scientific study is voluntary and that I can withdraw my consent at any time without giving reasons and without incurring any disadvantages.
